Key Responsibilities:
- Conduct clinical outreach to families, screen and assess prospective members for VIOP eligibility, and guide families through admissions.
- Coordinate care schedules including group therapy, individual therapy, family therapy, and psychiatric services.
- Maintain accurate documentation in EHR, engage in staffing meetings, and act as liaison between families, treatment team, and external providers.
Requirements:
- Master's degree in Social Work (MSW) or equivalent.
- Experience with intensive outpatient programs and pediatric populations.
- Proficiency with video tools and computer literacy.
Compensation & Benefits:
- Base salary range $70,000 annually.
