Continuing Care Specialist

Responsibilities:

• Guides clients and families in recognizing the value of continuing care.
• Builds trusted relationships with clients, families, and referral sources.
• Manages an active pipeline of continuing care opportunities.

Qualifications:

• At least one year of experience in the behavioral health field.
• Demonstrated ability to build trusted relationships across care settings.
• Strong attention to detail with the ability to accurately document client needs.
