Deliver medication-assisted recovery for substance use disorder via telephonic, text-based, and video-based telehealth.
Provide patient education on addiction, medications, and support services within a harm reduction framework.
Assess patient progress and collaborate with care team to develop personalized treatment care plans.

Responsibilities:

Coordinate with pharmacies, outside providers, and community agencies for seamless patient care.
Manage patient appointments, referrals, and report changes in status to prescribing clinicians.
Maintain HIPAA and 42 CFR Part 2 compliance with accurate documentation and schedule management.

Compensation and Benefits:

Pay range: $80,000 - $95,000 per year for full-time W2 position.
Benefits include 4 weeks PTO, 12 weeks paid parental leave, health/dental/vision, and 401(k).
Remote work with hardware provided; licensing and credentialing costs covered.
