Job Description
Care & Planning:
- Establish a care plan according to the physician's plan.
- Provide crisis intervention.
- Educate patients and caregivers.
Community Resources:
- Assist in community referrals for resources like public assistance and healthcare.
- Work with caregivers to resolve problems.
- Complete all required documentation.
Requirements:
- Master’s degree in Social Work.
- Licensed to practice in the state serviced.
- One year of social work experience in health care setting.
