What You’ll Do:

• Conduct targeted telephonic outreach to patients from provided lists, delivering high standards of customer service.
• Complete structured screening forms capturing demographics, chronic disease history, and functional decline indicators.
• Educate patients and families on palliative care, translating complex concepts into accessible language.

Your Qualifications:

• Current CMA or CCMA certificate with 2 years of healthcare experience required.
• Experience with motivational interviewing and patient education in serious illness topics.
• Strong organizational skills and proficiency with Microsoft and Google applications.

Additional Details:

• Willing to work Monday through Friday, 8:30am–5pm Eastern Standard Time.
• Background screening and vaccination required for patient-facing roles.
