Nurse Practitioner, Neurology and Memory Care

Role Overview:

• We are seeking a skilled and compassionate Nurse Practitioner licensed in Virginia to join our virtual care team.
• This is a remote, patient-facing clinical role delivering care to Local Infusion patients and partnering with physicians.

Job Description:

• Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth.
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
