About the Role:
- Serve as a central figure in patient care, navigating the healthcare journey for optimal outcomes.
- Strategically assess, plan, and facilitate comprehensive care across the continuum.
- Advocate for patients while collaborating with physicians, insurers, and post-acute providers.
What You'll Do:
- Work with at-risk patients on self-management support and create action plans.
- Lead an interdisciplinary team in managing high-risk patients, reducing preventable admissions.
- Assist patients and families in navigating the healthcare system to minimize fragmentation.
Qualifications:
- Clear and current CA RN license and two years relevant experience.
- Strong clinical acumen, communication, and organizational skills.
- Ability to work autonomously and handle multiple priorities.
