- Provide virtual patient support and guidance to individuals managing chronic kidney disease.
- Work with a collaborative team to drive goals of care discussions and advance care planning.
Qualifications:
- Master's degree in Social Work and current California LCSW licensure required.
- At least one year of hospice or palliative care experience and 6-8 years of social work experience.
