Physician Liaison

Provider Partnership & Relationship Management:

• Build and maintain trusted, collaborative relationships with assigned providers (physicians, practice administrators, and staff).
• Conduct regular interactions (both virtual and in-person) to review performance, co-develop improvement plans, and address barriers to change.
• Serve as the primary point of contact and advocate for assigned providers, representing their needs and perspectives internally.

Education & Enablement:

• Engage with practice administration, staff, and providers to support workflow improvement, education, and awareness on value-based care program requirements, quality measures, and performance incentives.
• Collaborate with internal teams to resolve barriers that may inhibit provider adoption.

Performance Analysis & Insights:

• Translate complex data into clear, actionable narratives tailored to each provider and/or practice.
• Monitor progress on initiatives, track leading indicators of behavior change, and adjust strategies based on results.
