The Role:
- Step away from traditional clinic walls to deliver high-impact, compassionate care to frail, elderly, and medically complex patients in their homes.
- Use advanced mobile diagnostics to evaluate and treat acute symptoms, designing targeted plans and reducing avoidable hospitalizations.
What You'll Do:
- Conduct comprehensive telehealth evaluations and guide in-home interventions in real time with paramedics.
- Collaborate with an interdisciplinary team and maintain accurate EHR documentation for seamless care.
Qualifications:
- Board certification in Emergency Medicine, Internal Medicine, or Family Practice and an active Pennsylvania or Louisiana medical license.
- Preferred experience in value-based care, telehealth, and working with emergency medical services.
