What You'll Do:
- Conduct comprehensive nutrition assessments and develop personalized care plans.
- Deliver telephonic nutrition counseling and education for chronic disease management.
- Coordinate with interdisciplinary teams to ensure holistic, patient-centered care.
What You Bring:
- Registered Dietitian Nutritionist (RDN) with a Bachelor's degree in nutrition or related field.
- Minimum 3 years of clinical nutrition experience; case management experience preferred.
- Proficiency in Microsoft Office and ability to work independently in a remote setting.
