Physician Partner

Role Overview:

• We are seeking a mission-driven Physician Partner to integrate health-related social needs into care delivery for older adults.
• This is a high-impact, part-time remote role with flexible hours, offering the chance to shape clinical workflows from the beginning.

Key Responsibilities:

• Conduct virtual visits and review medical history for older adults.
• Identify health-related social needs and create care plans.
• Collaborate with Community Care Advocates and operational teams to refine workflows.

Required Qualifications:

• Active, unrestricted MD or DO license in good standing with Board certification in Internal or Family Medicine.
• Active Florida state medical license, Medicaid credential, and Medicare enrollment required.
• Minimum 2 years post-residency clinical experience and prior telemedicine experience strongly preferred.

Preferred Qualifications:

• Licensure in additional states and fluency in Spanish are a plus.
• Startup mindset with adaptability and cross-functional communication skills.
