Role:
- Provide call-based care to members in the Remote Patient Monitoring (RPM) program.
- Evaluate medical history and data to determine program eligibility, serving as primary clinical point for escalations.
- Collaborate with RNs, RDs, and Accountability Coaches to reinforce clinical goals and SMART goal progression.
Qualifications:
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) with active NP certification.
- Valid unencumbered multi-state nurse practitioner license; 3+ years managing diabetes patients.
- Familiarity with CGMs, behavior change theory, and Spanish-speaking highly preferred.
