About the Role:
- Cover weekend tele visits for home health patients in the SoCal/OC region.
- Conduct remote assessments daily 5PM to 11PM.
- Act on Circadia platform alerts to ensure continuity of care for high-risk patients.
What You’ll Do:
- Perform after hours remote patient care with follow-up and urgent telehealth assessments.
- Convert data into action by interpreting platform insights and prioritizing high-risk patients.
- Efficiently complete tele visit management for designated patients, focusing on alerts and high-acuity cases.
How Success is Measured:
- Achieve 25+ after hours tele visits per day.
- Maintain time to acknowledgment rate under 24 hours for flagged patients.
- Complete all notes using AI-powered tools within 24 hours of each encounter.
