What You’ll Do:
- Conduct initial and follow-up telehealth assessments on flagged patients.
- Initiate/adjust plans of care and coordinate with referring and primary care providers.
- All via secure video/audio platform.
Data into action:
- Interpret trend-based insights from the Circadia platform.
- Prioritize high-risk patients and drive timely interventions to prevent decline.
- Prevent unnecessary ED visits.
Compensation & Perks:
