Provides telephonic assessments of members’ medical, psychosocial, physical and spiritual needs. Develops, implements, and monitors person centered service plan, and ensures continuity of care across all healthcare settings. Collaborates with the interdisciplinary team, PCP, member/member’s representatives, and specialists. Utilizes benefit structure and community-based resources to meet member’s needs.

Expanding autism services and need Board‑Certified Behavior Analysts (BCBAs) to serve as part‑time consultants. You will help us shape local care models, expand insurance contracts, and if interested, create a launch‑pad to build or grow your own independent practice—without the usual operational headache.
