As a Care Coordinator, you will serve as the primary point of contact for partnerships, facilitate internal and external referrals for Intensive Treatment Services, and track the lifecycle of all referrals to ensure timely outreach. You will also communicate directly with clients and coordinate with Foresight staff to ensure comprehensive care. This role requires strong organizational, communication, and documentation skills.
Complies with the Official Coding Guidelines in ICD-10-CM and CPT coding conventions to assign for diagnoses and procedure codes for Fred Hutch outpatient facility services. Clinical judgment and high level analytical skills are utilized to code and abstract the legal medical record to support appropriate reimbursement.
