The Vice President of Care Transformation will lead a team to help customer organizations plan, build, and support Community Health Worker programs at enterprise scale using IMPaCT’s evidence-based infrastructure. Responsibilities include overseeing implementation, conducting gap analyses, defining key performance metrics, and supporting electronic health record integration and billing pathways. The VP will manage a growing team and ensure effective performance and account management.
