Deliver virtual individual and group counseling sessions that support recovery and promote wellness. Conduct intake assessments to evaluate Member readiness and determine appropriate level of care. Facilitate group and one-on-one counseling sessions using Affectβs evidence-based curriculum. Provide crisis support and coordinate appropriate referrals when needed. Maintain timely, accurate documentation in our electronic medical record (EMR) system. Collaborate with clinical supervisors on individualized treatment and discharge planning.

Play a critical role in Pair Team's whole-person, interdisciplinary care model by providing mental health resources and therapeutic interventions for individuals living with Serious Mental Illness, Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs.
