The RN Care Manager collaboratively works with a team to review lab results, coordinate care, and educate patients. This role involves initiating care plans, assessing patient needs, and utilizing community resources, all while ensuring comprehensive care and adherence to treatment plans.
This casual, part-time hybrid role requires a motivated candidate to virtually triage the waiting room of high volume ICC’s, initiate protocols, and improve throughput, with the option to work remotely or from one of the 25 ICC locations. The position involves 12-hour shifts every two weeks, including weekends and holidays, and requires in-clinic orientation. Prior emergency department or urgent care nurse experience is preferred.
