Responsibilities:
- Conduct around 20 telehealth visits daily for homebound patients.
- Review and sign care plans for each patient annually.
- Supervise a pod of nurse practitioners and physician assistants.
Compensation:
- Per-activity rate card with total annual target of $240,000-$270,000.
- Per-visit and ancillary encounter rates, plus bonuses.
- Includes CME reimbursement and licensure support.
Qualifications:
- MD or DO with active NJ license, board certified in IM, FM, or Geriatrics.
- Comfortable with high-volume telehealth and efficient documentation.
- Prefer experience in home-based primary care or supervising APPs.
