About the role:
- The Behavioral Health Care Manager delivers evidence-based treatment via telehealth to oncology patients.
- You will work within a Collaborative Care Model, consulting with a psychiatrist and communicating with oncology teams.
What you will be doing:
- Conduct assessments, develop treatment plans, and implement therapy modules.
- Maintain a patient registry and track progress to guide care decisions.
- Participate in weekly consultation meetings and ongoing training.
