About the Role:
- Provide virtual care via CBT to eligible cancer patients.
- Collaborate with multidisciplinary care team including Health Partners, Nurses, and Nurse Practitioners.
- Conduct initial needs assessments and create care plans.
Qualifications:
- Master of Social Work and active LCSW license.
- At least two years of clinical work experience, adept at CBT.
- Comfortable with Slack, Zoom, Google Suite, and EHR systems.
Additional Information:
- Fully remote position with hours Monday-Friday 8:30 AM to 5 PM ET.
- Compensation range $75,000 to $95,000 annually depending on location and experience.
