Your Role:
- As a Pharmacist, you ensure the best clinical care for patients through medication management.
- You collaborate with a multidisciplinary team including physicians, nurses, and social workers.
- This fully remote role requires working 8:30am–5pm Central Time.
Role Responsibilities:
- Consult with patients and caregivers to implement pharmacy care management programs.
- Optimize medication regimens using CMM principles and drive quality initiatives.
- Analyze cost-savings strategies and act as an internal subject matter expert.
