New Therapy Management Clinical Pharmacist

Job Summary:

• The Therapy Management Clinical Pharmacist provides pharmaceutical care services remotely, collaborating with healthcare professionals to ensure safe and effective medication therapies.
• The pharmacist performs phone-based assessments and focuses on identifying, resolving, and preventing medication-related problems and adherence barriers.

Job Duties:

• Review and evaluate medication orders for accuracy, appropriateness, and safety.
• Provide medication counseling and collaborate with healthcare providers to develop therapy plans.
• Monitor drug interactions, adverse effects, and maintain accurate patient records.

Experience/Education:

• Applicant must hold a Doctor of Pharmacy degree and be a licensed pharmacist in New York.
• Experience in diabetes mellitus and/or weight management is required.
