Accountability Coach

About The Role:

• We are seeking highly enthusiastic, self-motivated professionals passionate about remote care delivery.
• In this role, you will support diabetes education and management for our members through lifestyle modifications and high-frequency support.
• You will partner with clinical professionals to ensure members receive the attention and education they need to lead healthier lives.

Key Responsibilities:

• Serve as an expert diabetes health coach, supporting individuals to achieve the best health outcomes possible.
• Engage members with empathy and care via telephone calls and text messaging.
• Deliver education and personalized guidance according to Perry Health protocols to help members better self-manage their condition and adhere to their care plan.

Qualifications:

• National Board-Certified Health & Wellness Coach (NBC-HWC), OR LPN or higher clinical licensure is required
• Motivated learner, willing to acquire proficiency with the necessary technology (computers, software applications, phone systems); experience or training in Motivational Interviewing to support sustainable behavior change.
• Creative thinking, an eye for detail, ability to work independently, a positive attitude, and the ability to excel in a fast-paced atmosphere while collaborating with diverse groups at all levels of the organization.
