Care Coordinator, MA (Bilingual)

Your Role:

• Support proactive care coordination for patients with complex chronic conditions including CKD.
• Conduct high-volume text and phone outreach to engage patients and reinforce care plans.
• Maintain audit-ready documentation and ensure HIPAA compliance.

What You'll Do:

• Enroll and onboard patients into CCM programs, verifying eligibility and consent.
• Coordinate referrals, care transitions, and follow-up appointments with providers and partners.
• Use EHR and CCM platforms daily to manage patient messaging and collaboration.
