About the Role:
- Own continuity of care for patients needing more than virtual outpatient therapy.
- Work remotely from anywhere in the US as a full-time W2 team member.
Key Responsibilities:
- Identify high-acuity patients and assess their needs, preferences, and coverage.
- Build individualized transition-of-care plans and connect patients to appropriate services.
- Execute referrals, schedule appointments, transmit records, and confirm attendance.
What We're Looking For:
- Calm, relentless problem-solvers with 2+ years of healthcare case management experience.
- Strong communication skills and comfort with telehealth platforms and EMRs.
