About the Role:
- Provide high-quality virtual care via telehealth to patients and families living in home settings.
- Serve as the primary point of contact for caregivers, families, and support networks.
- Conduct biopsychosocial assessments and develop personalized care plans.
Key Responsibilities:
- Refer and connect families to community-based services and supports.
- Communicate with primary care physicians and specialists to ensure continuity of care.
- Document all interactions, including presenting issues, goals, interventions, and action plans.
Experience and Skills:
- Preferably 2 years of experience in a healthcare or community-based setting.
- Demonstrate cultural humility and ability to interact with diverse populations.
- Experience with EHRs and strong computer skills are a plus.
