Care Navigator

Summary:

• The Care Navigator supports providers and care managers with non-clinical tasks such as patient inquiries, scheduling, and EHR management.
• This is a 100% remote position requiring a reliable high-speed internet connection.

Key Responsibilities:

• Address patient and DPOA inquiries via telephone and the patient portal.
• Manage electronic health records, maintain patient rosters, and facilitate provider orders and authorizations.
• Collaborate with the interdisciplinary care team to ensure seamless communication.

Qualifications:

• High school diploma or equivalent with 1+ years of medical office or related experience.
• 1+ years of EHR documentation or practice management tools.
• Knowledge of medical terminology and ability to interpret medical records and lab results.

Work Environment:

• 100% remote position in the Central Standard Time Zone.
• Requires a distraction-free environment and excellent organizational skills.
