About the Role:
- This is a full-time, temporary remote position focused on high-volume outreach and end-to-end care coordination.
- You will help patients navigate complex systems, overcome barriers, and access care that improves their health and well-being.
Key Responsibilities:
- Conduct patient assessments to understand clinical, social, and logistical barriers to care.
- Educate patients on available healthcare services and community resources, and follow up to ensure completion of care.
- Maintain accurate documentation of all interactions in accordance with HIPAA and privacy standards.
What We're Looking For:
- 4+ years of experience in care navigation, coordination, social work, call centers, or patient outreach.
- Strong critical thinking, emotional intelligence, and ability to work independently in a fast-paced environment.
- Flexibility, humility, and a sense of humor are key personality traits for thriving on this team.
