About the Role:
- Frontline Patient Support Specialist helping patients understand and enroll in Phamily-enabled care programs.
- Conduct high-volume phone outreach and manage the Scheduling inbox with timely follow-up.
Key Responsibilities:
- Educate patients on proactive chronic care support and address questions or concerns.
- Accurately document patient interactions, outreach attempts, and enrollment outcomes.
- Collaborate with care teams and internal partners to ensure a seamless patient experience.
Requirements:
- Bilingual English/Spanish with prior care management experience.
- Strong call center and telephone communication skills.
- Comfort using technology, with reliable internet, personal laptop, and smartphone.
