Care Navigator - Licensed Nurse

Responsibilities:

• Manage physiological markers like blood pressure and blood glucose with clinical appropriateness.
• Perform monthly wellness calls with assigned patients to define health goals and provide preventive education.
• Meet patient engagement program goals and follow appropriate escalation pathways for urgent care needs.

Requirements:

• Unrestricted RN/LPN/LVN license in a compact state, with Illinois preferred and ability to work CST hours.
• 1-3 years of clinical, patient management, or disease management experience, ideally with Medicare patients and virtual visits.
• Experience with motivational interviewing, health behavior change, or health coaching certification is a plus.

Program Context:

• The role operates within a Remote Patient Monitoring (RPM) program to reduce emergent care risks through early intervention.
• Encounters are performed via phone using a platform that receives electronically transmitted patient data.
• The candidate will be an integral player in developing processes and best practices for this new clinical program.
