Care Coordination:
- Book virtual consultations with doctors.
- Support patients who do not require doctor intervention.
- Refer to appropriate services including ER, mental health, and social services.
Documentation & Professional Practice:
- Maintain accurate electronic records and meet SLA targets.
- Maintain active SANC registration and participate in continuous professional development.
- Engage in quality improvement and team activities.
