Serve as the Clinical Communication Hub:
- Act as the first line of response for NPs, managing facility communications.
- Conduct telephonic triage for acute, non-emergency symptoms, escalating to the NP.
- Provide care plan education and clarification on the NP's behalf.
Own the End-to-End Specialist Referral Process:
- Manage the specialist referral lifecycle, from initiating outreach to tracking status.
- Compile and send clinical documentation to specialist offices.
- Coordinate patient transportation with facility staff.
Drive Clinical Data & Quality Insights:
- Locate and synthesize critical patient information from Health Information Exchanges (HIE).
- Support program-specific tracking (e.g., Chronic Care Management (CCM) enrollment, consents, and time).
- Perform data abstraction from the EHR to support quality dashboards.
