Determine the most appropriate level of care for each client, ensuring alignment with their clinical needs and treatment goals, and communicate program expectations. Collaborate closely with the clinical treatment team, providing detailed assessment findings to support individualized care planning, maintaining accurate and compliant documentation of all assessments.
