About the Role:
- We are building on the GUIDE model to diagnose dementia and facilitate care navigation earlier.
- You will take the lead to meet patients and families wherever they are in the dementia diagnosis journey.
- You will work closely with Tembo’s Head of Medical to refine and develop workflows.
What You'll Do:
- Conduct virtual assessments of patients with suspected dementia diagnoses.
- Conduct medication reviews and develop comprehensive care plans.
- Coordinate care alongside care navigators and patient’s primary care teams.
