Licensed Clinical Social Worker (Texas)

Responsibilities:

• Conduct comprehensive assessments, including psychosocial distress, mental health needs and social determinants of health risks.
• Clinically interpret assessment results to identify priority needs, emerging risks, and appropriate interventions.
• Conduct serious illness conversations with patients and families; facilitate advance care planning.

Requirements:

• MSW from an accredited program, with an active LCSW license in Texas.
• Experience working within an interdisciplinary care model.
• Proficiency in using electronic health records (EHRs) and digital tools.
