Social Worker

What you’ll do:

• Conducts comprehensive psychosocial assessments to identify patient and family strengths
• Provides grief and bereavement support to patients and families
• Screens for and addresses mental health concerns including depression and anxiety

Your experience and qualifications:

• 2+ years of healthcare social work experience
• Experience in hospice, palliative care, oncology or home health
• Strong understanding of cultural competency with the target population
