Role Description:
- Be the primary point of contact for ECM members with behavioral health needs.
- Provide relationship-based outreach and care navigation.
- Offer brief skills-based behavioral health support.
Responsibilities:
- Manage a caseload of ECM members with complex behavioral health needs.
- Conduct face-to-face visits.
- Maintain engagement using evidence-based approaches.
Qualifications:
- Fluency in English and Spanish.
- Active Clinical Social Work License (LCSW).
- Experience supporting members with behavioral health needs.
