The Licensed Clinician is a core member of the Collaborative Care team involving the patient's primary care provider, psychiatric consultant, peer recovery support specialist and other behavioral health providers. Beyond clinical care, responsibilities will include team management and supporting colleagues. The Licensed Clinician provides evidence-based interventions using the Collaborative Care Model (CoCM) to promote patient recovery goals.
The Trauma Clinical Care Navigator will provide initial assessment, support, referral, and ongoing care coordination for members within Spring Health’s innovative Trauma program, offering screening, brief intervention, and referral of members to a trauma specialty treatment provider.
The Behavioral Health Care Manager is the primary point of contact for patients who are enrolled with April Health, responsible for coordinating the patient’s care with the primary care clinician and consulting psychiatrist, tracking the patient’s symptoms with validated measurement tools, and directly meeting with the patient to deliver evidence-based treatments.
