Responsibilities:
- Coordinate facilitation of patients' specialty drug treatment, ensuring timely delivery to the appropriate site of care.
- Review chart notes and perform utilization management on a patient/drug specific case basis.
- Handle case assignments, review case progress, properly document case notes, and determine case closure.
Qualifications:
- PharmD required; current unrestricted licensure as a Registered Pharmacist in state of residence or ability to gain reciprocity.
- Retail, hospital, or specialty pharmacy experience required; utilization and case management experience preferred.
- Expert in Microsoft Office Suite.
