Responsibilities:

  • Provide medication management services inclusive of medication access, medication education, medication adherence, comprehensive/directed medication management, and quality metric support.
  • Assist with medication access management services between the patient, pharmacy, insurance, and medical provider.
  • Document encounters in the patient electronic medical record, including closed-loop communication with patient care team.

Qualifications:

  • Graduate of an accredited college/university of pharmacy (Pharm.D) required.
  • Completion of ASHP accredited PGY-1 Pharmacy Residency in pediatric or ambulatory care required.
  • Active license in Washington strongly preferred; bilingual English/Spanish preferred.
