Responsibilities:

  • Provide support to patients and families experiencing a behavioral health crisis and handle case referrals, assessments, and progress reviews.
  • Coordinate care with agencies like DCFS and school systems, and collaborate with an interdisciplinary team to facilitate treatment plan goals.
  • Assess social determinants of health, connect families to resources, and participate in scheduled consultations to monitor patient progress.

Qualifications:

  • Master’s degree in Social Work (MSW) is required, with bilingual English/Spanish skills preferred.
  • Need 2+ years of case management or relevant experience in child & adolescent care, school-based settings, or community mental health.
  • Must have experience with acute behavioral health needs, team-based care, and excellent knowledge of community resources.
