Responsibilities:
- Cover Transitions of Care program needs.
- Conduct telephonic outreach and video/phone visits for recently discharged patients.
- Focus on medication reconciliation and symptom screening.
Requirements:
- Certified Nurse Practitioner in Adult, Family or Geriatrics.
- Active state licensure in MA and/or CT.
- 5+ years of post-graduate clinical experience in Primary Care or Geriatrics.
