As a Healthguide, you’re serving as the vital bridge between patients and their care teams—helping people overcome barriers, navigate their health journeys, and live healthier, more confident lives. You will be building trusted relationships that empower patients to take control of their health. Engaging through calls, texts, and emails to connect patients with the right care, close care gaps, and solve real-world health challenges. Partnering with physicians, nurses, and care teams to coordinate care and improve outcomes. In addition, capturing each interaction accurately in the electronic health record (EHR) to ensure seamless communication. Supporting medical practice partners while contributing to a culture of empathy, accountability, and continuous learning.
