Bilingual Licensed Clinical Social Worker (Remote)

Pair Team is seeking a passionate Behavioral Health Care Manager to change primary care operations for those who need it most. As a Behavioral Health Care Manager, you will provide mental health resources and therapeutic interventions for individuals living with Serious Mental Illness, Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. You will use evidence-based approaches, provide crisis intervention, educate individuals on behavioral health conditions, and collaborate on care issues with the Enhanced Care Management team. The position is primarily remote with occasional on-site visits.
