About the Role:
- You will be a key member of an integrated Care Team providing oncology care navigation.
- You will deliver direct patient care by phone and virtual platforms.
What You'll Do:
- Provide triage, case management, and supportive care across the cancer journey.
- Collaborate with clinical and non-clinical partners to address patient needs.
What We're Looking For:
- Bachelor of Science in Nursing and multi-state RN license required.
- At least 3 years of nursing experience, including 2 years in oncology.
