Responsibilities:
- Conduct timely post-discharge follow-up calls.
- Review discharge medications and escalate concerns.
- Coordinate and/or schedule follow-up appointments.
Requirements:
- Certified Emergency Medical Technician (EMT) or Medical Assistant (MA).
- 3-5 years of experience preferred.
- Outpatient healthcare experience is strongly preferred.
Additional Skills:
- Experience with electronic medical records (EMR) systems.
- Proficient in basic technology platforms.
- Strong communication skills.
