Senior Director, Care Coordination Operations

Porter is seeking a Senior Director of Care Coordination Operations (Care Management, Disease Management, Utilization Management) who is a strategic and operational leader. The Senior Director is responsible for delivering high-quality, efficient, and compliant care coordination services. This role oversees all aspects of Porter’s Care Coordination services, while leading the Care Guide Team to ensure exceptional member and client outcomes. Key responsibilities include leading the development, implementation, and continuous improvement of care coordination capabilities; overseeing the Care Guide Team, fostering a culture of accountability and collaboration; ensuring seamless integration of programs with organizational priorities and regulatory requirements; developing data-driven performance metrics; collaborating cross-functionally; staying current on regulatory changes and industry best practices; providing leadership and mentorship; and serving as a key partner to client stakeholders.
